The checklist includes guidance on gathering:
- Current list of medications
- Allergies and/or dietary restrictions
- Emergency contact information
- Primary care physician details
- Living Will
- Advance Healthcare Directives
- Medical Power of Attorney and HIPAA authorization forms
- Health insurance information
